Who we are

A dynamic team committed to excellence in providing accessible and timely diabetes education and self-management support to adults with diabetes, prediabetes or those at risk.

Our team consists of Registered Dietitians, Registered Diabetes Nurse Educators, and a Social Worker providing accessible, timely diabetes education and self-management skills to adults 18 years of age and older with Type 1, Type 2, Gestational, Prediabetes or those persons at risk of diabetes..

Social Work Counselling

  • Improving one’s quality of life and general well being
  • Developing Psychosocial assessments and treatment plans
  • Implementing therapeutic interventions such as Cognitive Behavioral Therapy
  • Addressing and sorting out complex client needs (e.g. legal, financial, housing, food security, etc.)
  • Providing assistance completing challenging paperwork such as Trillium and Ontario Disability Support Program (ODSP) applications
  • Advocating stress management strategies while emphasizing excellent self-care techniques

Do I need a referral?

We accept self-referrals as well as referrals from your family doctor or specialist. Click here for the Referral Form

Do I have to pay?

The program is free of charge as long as you have OHIP coverage.

The Diabetes Education Program (DEP) provides both one on one appointments and group workshops on a variety of topics including:

  • Blood sugar control
  • Weight management
  • Physical activity
  • Managing your medications
  • Insulin initiation and adjustment
  • Foot care education
  • Healthy eating
  • Grocery store tours
  • Stress/anxiety management
  • Financial support programs

Should I attend?

People with diabetes should take an active role in the day-to-day management of their diabetes.

We will help you gain the tools, skills, and confidence to best self-manage your diabetes.

Attending a Diabetes Education Program can help you self-manage your diabetes and connect with other people living with the same condition.  You will have the help of a team who can answer your questions and provide encouragement on your journey to better health.
